To save on time, please feel free to download and fill out our new patient intake forms found below. These forms are required before we can offer healthcare services. Additionally, please bring a list of the medications you currently take.

Come to Our Office

Come and visit our office in downtown Portland. You can park at Star Park (we validate for one hour) or take a bus or the Max. We will take a photo copy of your insurance card and ID for your chart. Please be prepared to pay any copay or coinsurance as prescribed by your insurance company. For patients without insurance, payment is expected at the time of the visit.

Help With Insurance

Although we are in-network with many health plans, there are some that consider us out of network. We are a small practice and we will make every effort to verify your insurance coverage prior to your visit. However, we ask that you review your insurance coverage because you are your own best advocate.

Even when we verify benefits prior to your visit, many insurance companies have a disclaimer that states “benefits cannot be guaranteed until the claim has been received”. Therefore, what they tell us is subject to change until the claim is submitted. Because of these disclaimers, we cannot guarantee that your insurance will cover your visit, even though we do our best to obtain this information. This is frustrating for everyone! Especially when it can take hours on the phone to be able to talk to a live person.

Please never assume that your insurance will cover all of your healthcare needs. Many plans have high deductibles, co-pays, co-insurance and other fees that may be your responsibility. We are happy to work with you to resolve any coverage questions.

Call Us

Yes, we validate for one hour at the Star Park Garage right across the street from our building.

Our front door to the street is locked. There is a keypad to the right of the door and you can enter the code: 036. The system will then call our office and once we confirm who you are, we will allow you to enter.

Masks are no longer required in medical offices, but we still recommend them for your safety, especially during cold and flu season.

When you arrive, the receptionist will ask to see your photo ID and insurance cards and there will be some intake forms for you to fill out. Once a chart is generated for you, you will be taken to an exam room where the doctor will see you. The doctor will do a detailed examination of your feet and lower extremities, including x-rays when appropriate. She will then discuss the potential diagnoses with you and go over a treatment plan. Some treatments can be performed on the same day.

Our office hours are Monday and Thursday, 8am to 5pm. However, the phones are forwarded to Dr. Lantsberger at the Gresham clinic on Tuesday and Wednesday.
